Patches of Love Donor Quilt
The LifeShare Patches of Love Donor Quilt is another way to honor the legacy of your loved one. This quilt commemorates the love and life of those who offered the gift of life through organ, eye and tissue donation.
Quilt squares are provided by the donor’s family and each square is unique and personal. Some have pictures, others sayings. All together, they represent the lives of those who have been so generous.
We continuously collect memories from donor families to create more Patches of Love Donor Quilts. Once we receive enough squares, we sew them together into a quilt and they are displayed in our offices as well as at special events.

Making Your Quilt Square
• Each quilt square must be a 7 ½” x 7 ½” square. The design must not exceed a 7” x 7” square to allow for a ¼” seam around each edge.
• Any square submitted larger than the dimensions stated will not be used and will be returned prior to quilting.
• Quilt squares should have a design or theme that best represents your family or loved one.
• The square should include your loved one’s name on the front of the square. Please be aware that these quilts will be on public
display.
• Do not “finish” the square with quilting or backing.
• Squares can be any color or can include such sentimental materials as your loved one’s baby blanket, high school jacket, tie,
or favorite sweater.
• You may use buttons, crayons, fabric, paint, markers, glitter, thread, yarn or other mediums on the square. You can also include
photographs, poems, quotes or other symbols of your loved one’s life.
• We invite you to submit a short story or description about the design chosen for the quilt square. Sharing the story behind the
squares makes these quilts all the more powerful.

Mail Your Memorial Quilt Square
• Please print and complete the Quilt Release Form, including your name, your loved one’s name, your contact information and if you
wish, a few words describing your square (this is not necessary for submitting a square).
• Consider taking a photo of your square so that you will always remember it.
• Place your square in a plastic bag for protection, and then place within a larger envelope, including your completed Quilt Release
Form.
• We suggest mailing your quilt square using an insured carrier or registered mail so you can be sure it reaches us safely.

No. Due to the overwhelming response to this project, we are asking families to send in one quilt square for each donor.
Can I submit a square for someone who gave the ‘Gift of Life’ several years ago?
