LIFESHARE DONOR REGISTRY INFORMATION

This area of the LifeShare Donor Registry will answer common questions you may have about registering online to be a donor. These questions and answers are specifically related to the online donor registry.
Question: Who can donate?
Answer: Anyone can be a potential organ, eye and tissue donor, from newborn to senior citizen. Eligibility is determined on a case-by-case basis at the time of death and may be affected by medical/social history, cause of death, etc.
Question: Where can I register to be an organ, eye and tissue donor?
Answer: Registration can take place at several different locations. You can register to be a donor online, at tag agencies when you get your license or state ID card. If you would like to sign up online start at LifeShareRegistry.org, and follow the directions.
Question: Can I choose what is donated?
Answer: Yes, on the online registry you can list any specific exclusions you have to any organ, eye and tissue you do not want to donate. You may also choose if you would like to just donate for transplantation or whether you would like to donate for transplantation and research. The decision is up to you.
Question: If I sign up to be an organ/tissue donor at a tag agent or the Department of Public Safety (DPS), do I also need to sign up online?
Answer: If you choose to be an organ, eye and tissue donor at a tag agent or DPS, when you get your license or state ID card, you will be added to the registry automatically. If you have any exclusions or would like to donate for transplantation and research, we encourage you to register online also.
Question: If I move, change email addresses, telephone number or change my mind about something concerning donation where do I make this change?
Answer: If you have signed up with a tag agent/DPS you would need to visit DPS or a tag agent to make changes. If you sign up online, after you go through all the steps, you will be given a registration ID number. This number, along with other personal information, will allow you to log into the registry site and make changes through the My Profile area.
Question: Do I need an email account to register online?
Answer: Yes, you need a current email account so you can receive your registration ID number and confirmation of your registration. You can sign up for free email accounts at yahoo.com, hotmail.com and mail.com.
Question: When registering online do I have to sign a signature card to finalize my registry?
Answer: When registering online, you will be required to check a box acknowledging the terms and conditions of your registration. By accepting the terms and conditions, you are providing your electronic signature to register.
Question: What happens if I am under the age of 18 and register online?
Answer: If you are under the age of 18 you are simply providing your intent to be a donor by registering. Oklahoma law requires your parent or legal guardian to give consent for donation until you reach the age of 18. However, once you turn 18 years of age, and if you are already in the online registry, your registration will change from indicating an intent to be a donor to an actual consenting adult donor.
