"Donate Life" License Plate

These unique plates were created during the 2004 legislative session and will help to raise awareness for donation.

The cost of a plate is $15 with a $3 mail fee. The Donate Life license plates were created to promote organ and tissue donation and to create awareness of the need for registered donors. LifeShare of Oklahoma does not profit from the sale of these license plates.

How to order your "Donate Life" license plate:

	Download an order form from this site or call 800-826-LIFE to receive a form by standard mail.
	Complete the entire form. Be sure to follow all instructions. Make certain to select the "Organ, Eye and Tissue Donor" option.
	Mail the form with payment to: Oklahoma Tax Commission Motor Vehicle Division, Special Plate Section 2501 Lincoln Blvd. Oklahoma City, OK 73194-0013
