"Donate Life" License Plate
These unique plates were created during the 2004 legislative session and will help to raise awareness for donation.

The cost of a plate is $15 with a $2 mail fee. The Donate Life license plates were created to promote organ and tissue donation and to create awareness of the need for registered donors. LifeShare of Oklahoma does not profit from the sale of these license plates.

How to order your "Donate Life" license plate:

Download an order form from this site or call 800-826-LIFE to receive a form by standard mail.

Complete the entire form. Be sure to follow all instructions. Make certain to select the "Organ, Eye and Tissue Donor" option, fourth from the bottom.

Donor Recognition Ceremonies
LifeShare of Oklahoma and the Oklahoma Lions Eye Bank honor organ, eye and tissue donors and their families in ceremonies in Oklahoma City and Tulsa. This year, ceremonies will be on August 2, 2-4 p.m., in Tulsa at the Renaissance Hotel and Convention Center. And on August 3, 2-4 p.m., in downtown Oklahoma City at the Hilton Skirvin Hotel.
